Retained Patient Value (RPV)

Our unique way of calculating the economic factor of a patient engagement service on a clinic.

Retained Patient Value card on the ekco Home tab, showing period total and trend

Definition

RPV = O + P + A

Over a rolling 30-day window, for patients in the engaged cohort. O is store order revenue, P is the visit value of new care plans, and A is the visit value of attributed bookings. Terms are additive and mutually exclusive: the same visit is counted at most once.

Plug in one 30-day window for your engaged cohort. Store orders (O) are dollars. New care plans (P) and attributed bookings (A) use your average visit value, same as the ekco Home tab methods note.

O: store orders-
P: new care plans-
A: attributed bookings-
RPV (30 days)-

Worksheet only. Live clinics see measured RPV on the ekco Home tab. Churn avoided (C) is not in the sum yet.

Terms

Each row is one priced term in the sum. Included terms enter RPV on Home. Deferred terms are defined so the boundary of the measure is explicit.

TermMeasuresObservationValuationStatus
O — Store ordersRevenue from clinic store purchases in the windowA completed purchase by a patient in the cohortTransaction price at sale; voids and cancellations enter as zeroIncluded
P — New care plansReturn-to-care, proxied by confirmation of a new planStaff confirm a new plan in the window (not a routine edit to an existing plan)Clinic average visit value, once per qualifying confirmIncluded
A — Attributed bookingsAppointments that follow an ekco messageAn outbound ekco message, then a non-cancelled appointment within 30 daysClinic average visit value, once per patient per windowIncluded
C — Churn avoidedExpected loss avoided relative to baseline attritionRequires an estimated attrition rate we can defend; not in RPV yetOmitted. No counterfactual placeholder enters the sumDeferred

The engaged cohort

RPV is conditional on engagement. Patient i is in the cohort for window T if and only if all three hold at the open of T:

  • Opted in at the start of T.
  • Completed onboarding before T.
  • Received at least one scheduled ekco message in the 30 days before T (check-ins, surveys, refill reminders, care-plan prompts, and similar cadence messages).

Valuing visits

P and A are valued at the clinic's average visit value (set when the clinic is configured). If that average is missing, P = A = 0 and RPV reduces to O for the period.

Valuing orders

O uses the price recorded on the sale. If that record is unavailable, the current catalog price is used. Voided and cancelled orders contribute zero.

Level and change

The Home chart plots cumulative RPV over the current window against the prior window. The percent change is (RPV_t − RPV_{t−1}) / RPV_{t−1} on the period totals.

Scope note

A booking is an appointment on the calendar that has not been cancelled. Attendance is not observed in this version, so A is valued at booking, not at show. Churn avoided (C) stays out of the sum until attrition can be estimated without inventing a figure.
